==== Cough ==== Codeine was once viewed as the "gold standard" in cough suppressants, but this position has since been questioned. Some recent placebo-controlled trials have found that it may be no better than a placebo for some causes including acute cough in children. As a consequence, it is not recommended for children. Additionally, there is no evidence that hydrocodone is useful in children. Similarly, a 2012 Dutch guideline regarding the treatment of acute cough does not recommend its use. Low dose morphine may help chronic cough but its use is limited by side effects.
Selegiline, also known as L-deprenyl and sold under the brand names Eldepryl, Zelapar, and Emsam among others, is a medication which is used in the treatment of Parkinson's disease and major depressive disorder. It has also been studied and used off-label for a variety of other indications, but has not been formally approved for any other use. The medication, in the form licensed for depression, has modest effectiveness for this condition that is similar to that of other antidepressants. Selegiline is provided as a swallowed tablet or capsule or an orally disintegrating tablet (ODT) for Parkinson's disease and as a patch applied to skin for depression. Side effects of selegiline occurring more often than with placebo include insomnia, dry mouth, dizziness, anxiety, abnormal dreams, and application site reactions (with the patch form), among others. At high doses, selegiline has the potential for dangerous food and drug interactions, such as tyramine-related hypertensive crisis (the so-called "cheese reaction") and risk of serotonin syndrome. However, doses within the approved clinical range appear to have little to no risk of these interactions. In addition, the ODT and transdermal patch forms of selegiline have reduced risks of such interactions compared to the conventional oral form. Selegiline has no known misuse potential or dependence liability and is not a controlled substance except in Japan. Selegiline acts as a monoamine oxidase inhibitor (MAOI) and thereby increases levels of monoamine neurotransmitters in the brain.

==== Conditioned or unconditioned? ==== As a result of their doctrinal development, the various sectarian Buddhist schools eventually became divided over the question of whether or not the very principle of dependent origination was itself conditioned (saṃskṛta) or unconditioned (asaṃskṛta). This debate also included other terms such as "stability of dharma" (dharmasthititā) and "suchness" (tathatā), which were not always seen as synonymous with "dependent origination" by all schools. The Theravāda, vātsīputriya and sarvāstivāda school generally affirmed that dependent origination itself was conditioned. The mahāsāṃghikas and mahīśāsakas accepted the conditioned nature of the "stability of dharma", but both held that dependent origination itself was unconditioned. The Dharmaguptaka's Śāriputrābhidharma also held that dependent origination was unconditioned.

Shortly after the discovery of the neutron in 1932, Enrico Fermi realized that certain rare beta-decay reactions immediately yield neutrons as an additional decay particle, so called beta-delayed neutron emission. Neutron emission usually happens from nuclei that are in an excited state, such as the excited 17O* produced from the beta decay of 17N. The neutron emission process itself is controlled by the nuclear force and therefore is extremely fast, sometimes referred to as "nearly instantaneous". Isolated proton emission was eventually observed in some elements. It was also found that some heavy elements may undergo spontaneous fission into products that vary in composition. In a phenomenon called cluster decay, specific combinations of neutrons and protons other than alpha particles (helium nuclei) were found to be spontaneously emitted from atoms.
5α-Reductase inhibitors are inhibitors of the enzyme 5α-reductase, and are a type of specific androgen synthesis inhibitor. 5α-Reductase is an enzyme that is responsible for the conversion of testosterone into the more potent androgen dihydrotestosterone (DHT). There are three different isoforms of 5α-reductase, types 1, 2, and 3, and these three isoforms show different patterns of expression in the body. Relative to testosterone, DHT is about 2.5- to 10-fold more potent as an agonist of the androgen receptor. As such, 5α-reductase serves to considerably potentiate the effects of testosterone. However, 5α-reductase is expressed only in specific tissues, such as skin, hair follicles, and the prostate gland, and for this reason, conversion of testosterone into DHT happens only in certain parts of the body. Furthermore, circulating levels of total and free DHT in men are very low at about one-tenth and one-twentieth those of testosterone, respectively, and DHT is efficiently inactivated into weak androgens in various tissues such as muscle, fat, and liver. As such, it is thought that DHT plays little role as a systemic androgen hormone and serves more as a means of locally potentiating the androgenic effects of testosterone in a tissue-specific manner. === Pharmacodynamics === As a glucocorticoid, dexamethasone is an agonist of the glucocorticoid receptor (GR). It is highly selective for the GR over the mineralocorticoid receptor (MR), and in relation to this, has minimal mineralocorticoid activity. This is in contrast to endogenous corticosteroids like cortisol, which bind to and activate both the GR and the MR. Dexamethasone is 25 times more potent than hydrocortisone (cortisol) as a glucocorticoid. Its affinity (Ki) for the GR was about 1.2 nM in one study. The activation of the GR by dexamethasone results in dose-dependent suppression of the hypothalamic–pituitary–adrenal axis (HPA axis) and of production of endogenous corticosteroids by the adrenal glands, thereby reducing circulating endogenous concentrations of corticosteroids like cortisol and corticosterone. Dexamethasone poorly penetrates the blood–brain barrier into the central nervous system due to binding to P-glycoprotein. However, higher doses of dexamethasone override the export capacity of P-glycoprotein and enter the brain to produce central activation of GRs. In conjunction with the suppression of endogenous corticosteroids by dexamethasone, this results in skewed ratios of activation of peripheral versus central GRs as well as skewed ratios of activation of GRs versus MRs when compared to non-synthetic corticosteroids.
